Notice on Online Application for Health Declaration Certificate for Non-Chinese Passengers

From December 1, 2020,foreign passengers departing from Georgia and flying to China are required to apply for a " Health Declaration Form " online.

People traveling from Georgia to China who tested negative for the novel coronavirus: Nucleic Acid and Anti-Body Blood (IgM) Tests performed within 48 hours before boarding (counted from the time of swab and blood sample collection) can apply for the Electronic Health Declaration Form by logging on the website of https://hrhk.cs.mfa.gov.cn/H5/ via computer or smart phone or scan the QR code below to enter the login interface. Then you must register on the site by e-mail. After logging in, complete the necessary information and upload photos of negative results of two coronavirus tests - Nucleic Acid and Anti-Body Blood (IgM) (both results are required, and the photos sent must be clear and contain the entire results). After submitting your application, the Chinese Embassy will conduct verification as soon as possible. After passing the verification, the applicant will receive a green QR code with the symbol "HDC" called the Health Declaration Form, which authorizes to board the plane. Electronic or paper version of the QR code should be presented when boarding the aircraft for verification.

During the transition period from November 20 to 30, 2020, foreign passengers traveling to China can submit an application for a "Health Declaration Form" both via the website and e-mail. From December 1, 2020, the Embassy will no longer accept applications for "Health Declaration Form " sent by e-mail.

Chinese citizens should continue to apply for a green QR code with the "HS" logo under the "International Version of Epidemic Prevention Health Code" mini program using the WeChat app.
